Please print the informed consent form and sign prior to your appointment, or read through in advance, ready to sign at the clinic.

An informed consent form need only be presented once per patient.

Please also ensure you have read the Patient Responsibility section of the guidelines below.

2. Patient Responsibility

Patients are asked to consider the following screening questions prior to arriving for the appointment, and discuss by phone any concerns before face to face contact:

  • Temperature >37.8c in last 2 weeks.

  • Persistent cough in last 2 weeks.

  • Loss/reduction of taste or smell.

  • Living with someone self- isolating.

  • Pregnant

  • >70 yrs

  • Runny nose or nausea.

Do you fall into any of the government identified risk groups:

  • Chronic respiratory diseases such as asthma, COPD, emphysema, bronchitis

  • Chronic heart disease or heart failure

  • Chronic kidney disease

  • Chronic liver disease such as hepatitis

  • Chronic neurological diseases such as Parkinsons, Motor Neurone disease, Multiple sclerosis, Cerebral palsy, learning disabilities

  • Problems with spleen such as sickle cell disease or removal

  • Weakened immune system as a result of disease such as HIV or Aids, medicines such as steroid tablets or chemotherapy

  • Is not seriously overweight with a BMI over 40

Patient agrees that they need to inform clinic of any change to medical status before attending appointment

Patient understands chaperone is subject to the same screening process

Patient consents to having temperature taken

Patient consents to wearing a face mask during consultation. Please bring your own or a single use face mask can be provided at clinic.

Patients consent to their details being given to the appropriate authority in the event of a contact tracing event.


3. Infection control procedures

Patients will have been triaged in terms of symptoms , self-isolation or contact with COVID-19 prior to coming to clinic.

The layout of the clinic has been adapted to maintain social distancing where possible.

Clinic furniture has been replaced with simple designs with wipeable surfaces and the removal of any unnecessary materials.

A cleaning protocol exists between each patient-contact which involves using sanitising product to wipe down commonly used surfaces.

Clinic staff will be wearing PPE as recommended by Public Health England (details displayed on clinic notice board)

Payment is taken by CARD ONLY.

A full clean of the clinic is done at the end of the clinic day.


4. Clinic Organisational Changes

It is requested that the patient does not arrive in the clinic prior to the appointment time.

Personal items brought into the clinic premises should be kept to a minimum.

On arriving at clinic the patient immediately uses the hand sanitiser made available in reception.

As a result of cleaning between patients, please note that use of the toilet facilities is only available during appointment times.

Payment is taken by CARD ONLY.

At this time patients are requested to wear light/ non restricted clothing for treatment.  The use of gowns and changing rooms will be temporarily suspended. It is suggested at the first opportunity the clothes worn in clinic are removed and washed at high temperature to reduce any transmission risk.
